If a user updates their username, as noted in the JSMF user guide, this behavior will occur. However, there should be an error message occurring as well telling the user about this behavior. What version are you running?
Users do not have the option to change username via SMF account settings. That field is uneditable. They can change their 'name' but that does not generate an error either for the user or in the forum error log.
However, your post has set me in a direction. I'll try some functions with a test user and see if I can find what's casuing the reoplication.
