Unable to login with mod_smf

I have SMF 1.1.2 bridged to Joomla 1.0.12 using the latest joomlahacks bridge with the fix for lost password added. CB version 1.0.2 is installed, and the cb plugin is loaded.
I am using the CB registration. If I use the CB login everything works OK. I would like to use the smf login listed above as it shows additional information about smf after you login. I had this selected in the jsmf configuration but it does not allow a login. You are directed to a blank page inside of the joomla site wrapper. (not sure if this is what is meant by blank page in some of the other threads.) The address that the login takes you to if used by itself takes me to the forum login page. (My forum is unwrapped)

I tried enableing error reporting and I do not get any errors on the page I am directed to but I get the following errors on my home page at the login module.

Any ideas?
Should this login module be working?
